Form Due Dates for Fall 2004

• Supervising Teacher Information Forms (turned in by University Supervisor) are due to the Assessment Coordinator by March 18, 2005.
• Supervising Teachers request midpoint/final TEFEE forms from the Assessment Coordinator at least one week before they are needed (NOTE: you cannot receive TEFEE forms until candidate and placement information has been entered into the Common Database for Field Experiences.
• The Teacher Education Field Experience Evaluations (TEFEE) for midpoint assessment (turned in by the University Supervisor) are due to the Assessment Coordinator by April 1, 2005.
• The TEFEEs for final assessment (turned in by the University Supervisor) are due to the Assessment Coordinator by May 4 (if reports are needed to generate grades) or May 11 (if no reports are needed for grades).
